The documents below are mandatory for students to complete. Please pay attention to any instructions that need to be completed to avoid data input errors by the Study Program.

◉ Kampus Merdeka. Participation Application Form.

1. After completing it, the form must be submitted to the IR Department Office before the Kampus Merdeka enforcement.
2. Students are also required to upload the form (in pdf format) to the following online form: MBKM Participation Application.

◉ Internship Control Sheet.

1. This form must be filled in and signed by the authorized official/supervisor at the internship agency.
2. This document MUST be completed by students who are undertaking the MBKM Independent Internship program by the UMM HI Study Program.

◉ Internship Assessment Form.

1. This form must be filled in and signed by the authorized official/supervisor at the internship agency.
2. This document MUST be completed by students who are undertaking the MBKM Independent Internship program by the UMM HI Study Program.
